Use of an Atrial Lead with Very Short Tip-To-Ring Spacing Avoids Oversensing of Far-Field R-Wave

چکیده
OBJECTIVE The AVOID-FFS (Avoidance of Far-Field R-wave Sensing) study aimed to investigate whether an atrial lead with a very short tip-to-ring spacing without optimization of pacemaker settings shows equally low incidence of far-field R-wave sensing (FFS) when compared to a conventional atrial lead in combination with optimization of the programming. METHODS Patients receiving a dual chamber pacemaker were randomly assigned to receive an atrial lead with a tip-to-ring spacing of 1.1 mm or a lead with a conventional tip-to-ring spacing of 10 mm. Postventricular atrial blanking (PVAB) was programmed to the shortest possible value of 60 ms in the study group, and to an individually determined optimized value in the control group. Atrial sensing threshold was programmed to 0.3 mV in both groups. False positive mode switch caused by FFS was evaluated at one and three months post implantation. RESULTS A total of 204 patients (121 male; age 73±10 years) were included in the study. False positive mode switch caused by FFS was detected in one (1%) patient of the study group and two (2%) patients of the control group (p = 0.62). CONCLUSION The use of an atrial electrode with a very short tip-to-ring spacing avoids inappropriate mode switch caused by FFS without the need for individual PVAB optimization. Left and Right Ventricle Leads Switch as a Solution for T Wave Oversensing - How a Good Idea Turned Out Bad
A 50-year-old male with a CRT defibrillator received inappropriate ICD shocks due to T-wave oversensing. Decreasing the sensitivity to avoid T wave oversensing was not an option due to a suboptimal R-wave sensing amplitude. We decided to re-plug the LV lead in the RV port and the RV lead in the LV port. This however led to intermittent phrenic nerve stimulation due to mandatory bipolar (tip-rin...

Aim of the present study was to investigate far field R-wave sensing (FFRS) timing and characteristics in 34 Myotonic Dystrophy type 1 (DM1) patients undergoing dual chamber pacemaker implantation, comparing Bachmann's bundle (BB) stimulation (16 patients) site with the conventional right atrial appendage (RAA) pacing site (18 patients). All measurements were done during sinus rhythm and in sup...

A CRT-D patient presented with loss of biventricular pacing associated with heart failure symptoms. The electrocardiogram showed sinus rhythm with alternating wide unpaced and narrower paced QRS complexes. Device interrogation showed T-wave oversensing on all paced biventricular beats, with the following sinus P-wave not tracked due to it falling in the post-ventricular atrial refractory period...
متن کاملT Wave Oversensing
T wave oversensing is quite a familiar phenomenon to anyone who has watched cardiac monitors for sufficiently long periods in the coronary care unit. The counter starts giving dual beeps for every cardiac cycle instead of one. If it persists during several cycles, the displayed rate goes up and the monitor sounds an alarm indicating 'tachycardia'. This occurs usually when the monitoring lead di...
